Joining the practice

Becoming a patient

Joining the practice as a new patient is very easy provided you live within our catchment area (see below), if you are unsure, please discuss with our reception team on 01722 333214.

What do I need to do?

Please download and complete these forms and attend in person, with ID*, to verify your registration:

What we need

Photo ID (This should be either a driving licence or passport). If you don’t have any photo ID then you will need to bring the full unabridged version of your birth certificate

Two proofs of address – dated within the last 3 months.

Practice catchment – please check you are residing within the practice boundary before applying to join the medical centre.

Our Catchment Area

We plan to close to new registrations in the following outlying areas: Middle & East Winterslow, Lopcombe Corner, Winterbourne Dauntsey & Winterbourne Earls, Upper & Middle Woodford, Stoford, South Newton, Wilton, Stratford Tony, Bishopstone & Nunton.

Patients who are already registered and are living within these areas may remain registered with the surgery.

Temporary Patients

How we can help

If you are a temporary resident living within our practice area, we can register you for short term care, for a maximum of three months. You must be registered elsewhere with a GP in the UK.

To use this service, you will need to complete a GMS3/99 (see link below) instead of a GMS1 and provide ID as detailed above.
